Campaign questions
No co-op. Six difficulty names. Do not assume you can pause with a button I have not verified, and do not assume the campaign is fully offline.
From my notes. I threw out a Game8 sentence about the pause menu because it names the MW3 2023 campaign. The rest of that page's difficulty list still matches other notes, so the names stay and the button does not.
Co-op, difficulty, length
There is no co-op campaign. Squadmates are NPCs. Difficulties listed: Recruit, Regular, Hardened, Veteran, Realism and YOLO. Clearing Realism unlocks YOLO. Story length reports sit around 6 to 10 hours. See campaign length.
Split-screen is a multiplayer feature in the editions article, not a campaign feature. Sixteen missions are indexed on the mission list.
Open questions
Activision has not said the campaign runs with the network unplugged. I will not write 'offline works'. Cloud saves and mission select after the credits were not in the pages I checked.
Multiplayer and DMZ do require a connection. That part is not in doubt.

Is there campaign co-op?
No.
Can I pause and change difficulty?
The button path was not verified for this game. Difficulty names are published. The menu steps are not.
Can I play the campaign offline?
Not confirmed. Do not plan on it.
