Setting
Korea setting
Hajin is fictional. It is written as a South Korean city near both the northern border and a Russian border in Wikipedia's note.
From my notes. I read the campaign page, the Hajin deep dive and the Aju Press and IGN Latin America pieces on how the peninsula was researched. I did not find a sourced explanation for a changed North Korean flag, so that claim is not on this page.
Places
The invasion starts after an EMP. The opposing force is modeled on the Korean People's Army and is called NKA in some copy and North Korean Army on multiplayer maps. Infinity Ward has said it lacked official northern material and used interviews, video and Korean staff review.
Other mission cities are New York, Paris and Mumbai. DMZ's map is also Hajin, with 13 named points of interest on the official deep dive. The list is on the DMZ guide.
The flag question
Searches ask why a northern flag looks different. I could not verify a design change from text sources. Until a frame shows the in-game flag next to the real one, this page will not claim the flag was altered.
Korean coverage also discusses the ethics of setting a shooter in a war that ended in an armistice, not a peace treaty. That debate is real. It is not a lore fact.
